OCSEF - Science Fair Checklist
ALL STUDENTS MUST COMPLETE THIS CHECKLIST
The rules and regulations of the OCSEF are intended to be consistent with all national, state and local laws and regulations, as well as those of the California State Science Fair and the Intel International Science & Engineering Fair.
These rules are designed to protect you, your work, those affected by your work, and the integrity of the science community. We want to be sure that you recognize and plan for safety issues and ethical issues before you start your project:
____ Does your project involve the use of animals? If YES, then you must download and complete the Certification of Humane Treatment of Live Vertebrate Animals before you begin your project work.
_____ Does your project involve the use of animal cells or tissues, including DNA? If YES, then you must download and complete the Certification of Vertebrate tissue Sample Source before you begin your project work.
_____ Does your project involve people (human subjects)? If YES, then you must download and complete the Certification of Compliance for Research Involving Human Subjects before you begin your project work.
_____ Does your project involve the use of potentially hazardous organisms, chemicals, substances, or devices (anything so labelled or which, if not handled properly, can cause injury)? If YES, then you must download and complete the Certification of Hazards Control before you begin your project work.
If you answered NO to all of the above questions, you do not need to complete an OCSEF Certification Form.
If you answered YES to any of questions 1 through 4, it is strongly recommended that you submit your project plan and Certification Form(s) to the OCSEF’s Scientific Review Committee (SRC) for review before you start your experimental work.
You may also submit the SRC project proposal form if you are in doubt about potential hazards associated with your project. OCSEF will review the form and advise you about any potential problems.
For each of the questions that you answer YES, you must complete the appropriate Certification Form(s) and have the form(s) signed by the required advisors. It is strongly recommended that you also submit your plans to the OCSEF Scientific Review Committee (SRC) for review BEFORE you begin your experimental work.
How to complete Certification Forms and SRC submission Forms before you begin your project.
- All of the forms required for the OCSEF SRC review are available on the OCSEF website (dropdown boxes: Registration > Certification Forms) in PDF format:
- Download the forms you need. Save them to your computer. Fill in the required information. Type the names in the appropriate boxes of all the people (including yourself) who will need to sign the forms.
- Print a copy of each completed form.
- Go the web page titled: SRC Submission: (http://www.ocsef.org/index.php?option=com_rsform&view=rsform&Itemid=167)
- Complete the Contact Information section.
- Upload the Project Proposal Form and your completed Certification Form(s) from your computer to the SRC Submission Form.
- Click “Submit” to send your information to the OCSEF SRC
After review by the SCR, you will receive a response by email. The SRC will review the project plan; they may or may not recommend changes to conform to OCSEF regulations; they may advise of potential safety or ethical issues, or make suggestions to better conform to experimental best practices.
Obtain the required signatures on the Certification Form(s).
Make copies of each signed form. Keep one set for your records, put one set with your research notebook, and submit one set with your project registration.
